Candidate Service Agreement

I hereby authorize Old State Staffing (hereafter “the Company”), its employees, agents, private investigators or any representative of the aforesaid company, to perform investigations into my background, past behavior, to my character, general reputation, and mode of living. In addition, I further authorize investigations of the following:

• Investigative Consumer Reports: I authorize the Company to perform investigative consumer reports that may include credit reports, criminal history or arrest records, workers’ compensation histories, motor vehicle records, employment and unemployment records, military records and other sources of information.
  
  
• Education: I authorize schools, colleges or other education institutions to release any and all information requested. This includes transcripts, grades, attendance records, and any other information requested.
  
  
• Employment: I authorize all former and current employers to release any and all information regarding my employment history. This includes all information contained in my personnel file, salary history, condemnations, and all other pertinent information. I further authorize my supervisors and other work associates to disclose their opinions and observations of my work habits, qualities, competency, and skills.
  
  
• Authorization to Release: I authorize custodians of the records of any agency, government agency, or company as described above to release such information upon request of any investigator, agent, or representative of the Company. I understand that any or all of these investigations or inquiries may be performed prior to and periodically throughout the duration of my employment. I also consent to enrollment in Old State Staffing’s Continuous Monitoring Program for any non-temporary placement (equal to or greater than 12 months). This program includes real-time alerts of changes to my background or criminal history, which may be reported to Old State Staffing and, as appropriate, to my employer. Enrollment will remain active for one (1) year from the start of my placement. I understand that clients may elect to continue monitoring beyond that initial term for an additional fee, and I agree to the continuation of such monitoring and disclosure if elected by my employer.
  
  
• 10-Panel Drug Screening: If requested by the employer, I give my permission to undergo a 10-Panel Drug Screening Test and I am aware that the results of all screenings will become the property of the Company and will become a permanent part of my application at the Company.
  
  
• Re-disclosure: I understand that the information requested is for the use by the Company and may be re-disclosed only as authorized by law. I understand that I have the right to request from the Company a written disclosure of the nature and scope of the investigation conducted that I authorized above.
  
  
• Indemnification: I indemnify, release, and hold harmless the Company, any agents of the Company, or others reporting to or for the Company, any investigators, all former employers, reporting agencies, and all those supplying references and character references, from any and all claims, defamation, demands, and/or liabilities arising out of, or related to, such investigations, disclosures, or admissions.
  
  
• Signature and Acceptance of Agreement: Copies and facsimile transmissions of this authorization that show my signature are as valid as the original release signed by me. Acceptance of the disclosures in this Candidate Agreement are required as part of the application process.

Candidate acknowledges that in the course of being referred for employment opportunities through Old State Staffing, they may receive or be exposed to confidential or sensitive information about Old State Staffing’s clients, including but not limited to names, addresses, employment needs, household details, family members, schedules, or security procedures (“Confidential Information”). Candidate agrees not to disclose, share, use, or otherwise disseminate any such Confidential Information to any third party without the prior written consent of Old State Staffing. This obligation applies regardless of whether the Candidate is ultimately hired by the Client.

Candidate further agrees that any notes, summaries, communications, or records relating to client information, or the placement process, shall be maintained securely and, upon request by Old State Staffing, returned or deleted.

This obligation shall survive termination of the referral process and any subsequent employment.
